Attached for reference, the Opt In Form (Seniors / Juniors) along with the “Personal Risk Assessment” and also the “Health Check Questions” which you should be asking yourself before attending any training / matches.
In addition, in line with England Netball guidance we have prepared our own LPNC Risk Assessment.
If, at any point, before, during or after, a LPNC session, you feel unwell, please contact your team COVID Officer.
If you do test positive for COVID-19 then please let me know using the details set out below.
Today or at any point in the last 7 days have you had:
- A fever?
- A new, continuous cough?
- Any loss or change to your sense of taste or smell?
- Has any member of your household or someone that you have been in contact with reported any of the above symptoms in the last 14 days?
- Has any member of your household or someone that you have been in contact with tested positive for COVID-19 in the last 14 days?
- Have you been notified by the NHS test and trace system that you are a close contact of someone with known COVID-19 infection in the last 14 days?’
As set out by England Netball, if you or anyone else in your household has symptoms, you should not attend any netball activity. If you have been told to self isolate by NHS test and trace, then of course please follow their guidance.
